Thank you for choosing to submit your loved one to be part of the AIDS Monument.
The monument is a place where families can honor their loved ones and help bring awareness to others. It is a symbol of the human spirit. The memories of your loved one will help a community rise above the cultural silence of AIDS.
The Names Submission process is open to everyone. New names will be added on an annual basis with their unveiling during Noche De Las Memorias, a special ceremony on World AIDS Day, December 1st.
In order to process the names submission application, The Wall-Las Memorias Project requests a $50.00 donation. If an individual is unable to meet the expense of the processing fee they may set up a payment plan or contribute volunteer work as an alternative

The Wall-Las Memorias Project (TWLMP) respects the wishes of the deceased and his or her loved ones. The deceased name will be publicly listed on the AIDS Monument, so we request that every individual submitting this application consider the wishes of the deceased and his or her loved ones. We also ask that the individual submitting this application provide us with the contact information for the deceased’s closet living relative, so that we can bring the deceased’s family into our community of hope and healing.
TWLMP retains the authority to decline requests for name placement, in its sole discretion.
All information on this form is public and will be used only for the purposes of the AIDS Monument and TWLMP.
